Stem cells are unspecialised cells that can develop into other cell types and help the body repair tissue. In medicine, the term covers a wide spectrum. At one end are well-established, life-saving treatments such as bone-marrow and blood stem cell transplants for certain blood cancers, which are standard care in major hospitals. At the other end are a large number of commercial 'stem cell' offerings, often using mesenchymal cells from fat or bone marrow, marketed for everything from arthritis to autism, multiple sclerosis and anti-ageing.
It is essential to separate these. The clinics most patients encounter in medical tourism are generally offering the second kind — and for most of those advertised conditions, the treatments are experimental and not approved by major regulators. That does not always mean they are useless, but it does mean the benefit is unproven and the risk-to-reward balance is uncertain.
This guide is deliberately conservative. The goal is not to discourage you, but to make sure you can tell a legitimate, evidence-based use from a hopeful sales pitch.
Who looks into it, and why people travel
Patients typically fall into two groups. The first have orthopaedic problems — knee or hip osteoarthritis, stubborn tendon injuries — and are exploring a cell-based injection as an alternative or delay to surgery. Here some clinicians use stem cell or marrow-derived injections with cautious, condition-specific rationale, though even orthopaedic use is still being studied.
The second group are patients with serious chronic or degenerative conditions who have exhausted conventional options. For them, the appeal is understandable: hope where mainstream medicine offers little. This is precisely the group most exposed to overstated claims, because desperation is easy to exploit.
People travel because such therapies are offered more freely, and sometimes more cheaply, in countries with looser oversight. That availability is a double-edged sword. The same lighter regulation that makes a treatment accessible can also mean weaker safeguards around cell quality, sterility and honest consent.
How it is done and how approaches differ
Most non-transplant stem cell treatments use mesenchymal stromal cells. These may be autologous — taken from your own fat or bone marrow, processed, then returned — or allogeneic, derived from donated tissue such as umbilical cord. Delivery ranges from a targeted injection into a specific joint to an intravenous infusion intended to act throughout the body.
The difference between a local injection and a systemic infusion matters enormously. A guided injection into an arthritic knee is a contained, plausible intervention. A systemic infusion claiming to treat a neurological or autoimmune disease is a far bigger claim resting on far thinner evidence, and infused cells do not simply travel to and rebuild damaged organs the way some marketing implies.
Processing is where quality is won or lost. Ask whether cells are prepared in a licensed, quality-controlled laboratory, how they are characterised and tested for contamination, and who is accountable for the product. Vague answers are a serious warning sign.
What it typically costs
Stem cell therapy is among the most expensive regenerative treatments. A localised orthopaedic injection sits at the lower end, while multi-session programmes of expanded-cell infusions can reach well into the thousands of euros, sometimes packaged with hotel stays and additional therapies that inflate the total.
Insist on an itemised quote: diagnostics and imaging, cell harvesting or sourcing, laboratory processing, the number and type of infusions or injections, and follow-up. Be especially wary of large, round-number packages for broadly defined 'cell therapy' aimed at chronic disease, which often bundle unproven add-ons.
Because most advertised uses are experimental, insurance rarely reimburses them. Assume you are paying out of pocket and judge the price against the published evidence for your exact condition — not against the outcome you are hoping for.
Choosing a clinic and stress-testing the promises
Look for internationally recognised accreditation such as JCI, TEMOS or ISO, and confirm the treating physician is a board-certified specialist in the relevant field. Ask where and how the cells are processed, and whether that facility is licensed and inspected.
Then pressure-test the claims with direct questions. Is this treatment approved by a major regulator for my condition, or is it experimental? What do peer-reviewed studies show for patients like me? What is the realistic best outcome, and what are the risks? A credible clinic will answer plainly, avoid guarantees of a cure, and document informed consent carefully. Heavy reliance on patient testimonials, dramatic before-and-after stories, or pressure to decide quickly are red flags.
Protect continuity of care. You should leave with full records and a follow-up plan you can share with your doctors at home, and you should be wary of any provider that discourages you from involving them.
Recovery, follow-up and realistic expectations
The procedure itself is often quick — an injection or an infusion — and most patients have only minor, short-lived discomfort, with some soreness at a harvesting or injection site. In that narrow sense recovery is easy.
The harder truth is about outcomes. For most systemic indications, durable benefit is not established, and any improvement may be partial, temporary or absent. Agree in advance how you and the clinic will measure whether the treatment worked, and over what timeframe, so you are not left interpreting vague reassurances.
Plan follow-up before you travel. Keep your home medical team informed, retain all documentation, and report any unusual symptoms — fever, swelling, breathing problems — promptly. Stem cell science is genuinely advancing, but for now the wisest patient is one who stays grounded, demands evidence and treats sweeping promises with healthy scepticism.
