The technique the surgeon selects. A lift is not one standard operation. How much skin is removed, how the tissue is reshaped and the resulting scar pattern vary with your anatomy and the goal. More extensive reshaping means longer operating time and longer anaesthetic cover.
Whether an implant is added. Some patients are told a lift alone will not give the fullness they are describing. Adding an implant means a device cost and more surgical time. It should never be blended into one undifferentiated price.
Anaesthetic and facility. Theatre time, the anaesthetist's fee and any overnight stay are priced separately, and move with the complexity of the operation.

Hidden costs to ask about
Garments. The NHS advises a sports bra worn around the clock for up to three months after breast enlargement, and clinics apply similar thinking after a lift. Ask whether garments are quoted.
Time off work. Breast surgery typically keeps people off work for a week or two, longer for physical jobs. Unpaid leave belongs in the total.
The nights before you can fly. The NHS advises avoiding flights "5 to 7 days after procedures such as breast surgery" because of clot risk, and the FCDO says to board only when you are fit to fly.
Travelling back. If revision is discussed later, seeing your original surgeon means flying again. BAPRAS notes that returning "could add hundreds of pounds as well as involve significant time away from work".
Insurance and repatriation. Standard travel insurance does not normally cover elective surgery abroad. Gov.uk guidance for patients in hospital abroad states that without cover for repatriation, "you'll have to arrange and pay for it yourself".
The assumption that the NHS will fix it. BAPRAS states the NHS will always look after you in a life-threatening emergency but "will usually refuse treatment for less serious complications or bad outcomes following cosmetic surgery". BAAPS agrees: return to the clinic abroad, or pay for corrections in the UK.
